Individual Personalized 1:1 Dietetic consultations
Whole health understands that not everyone is the same nor does everyone suffer from the same health conditions therefore we tailor dietary advice and care plans for each individual.
Screening for Malnutrition
Whole health can complete Resident Nutritional Status Screening and Assessments
We identify residents who are at risk of malnutrition or are malnourished and monitor changes to nutritional status using the accredited practicing Dietitian screening tool – SGA or MNA.
Conduct supplement reviews for individuals by reviewing the current supplements that are being used. This is used to help improve effectiveness, reduce unnecessary waste and help individuals improve nutritional status.
Nutrition Education and Training
Wholeness health can conduct menu reviews to ensure that menus in aged care facilities are meeting nutritional standards.
Provide nutritional education to all staff including - clinical care managers, nurses, personal care staff and kitchen staff on a range of topics including: managing malnutrition, nutrition screening – using the MNA tool prior to Dietitian assessment, nutrition and falls prevention, enteral feeding and nutrition and hydration at end of life etc.
Wholeness health can provide nutrition and hydration reviews to investigate all aspects of food preparation, delivery and nutrition, in order to understand what is working well, and identify any areas for improvements.
